These Terms & Conditions (the “Terms”) govern your use of the Relief Plus Wellness website located at reliefpluswellness.com and the wellness services we provide at our clinic. By accessing our website or booking an appointment, you agree to be bound by these Terms. If you do not agree, please do not use our website or services.
Relief Plus Wellness provides professional wellness services at 1403 Gerrard St E, Toronto, ON M4L 1Z5, including:
Wellness services including Brazilian Lymphatic Drainage, Thai Oil Massage, Deep Tissue Massage, Relaxation Oil Massage, Head Massage, Foot Massage, Hot Stone Therapy, Herbal Compress Ball Therapy, and Facial Cupping are not provided as Registered Massage Therapy (RMT) and are not eligible for insurance reimbursement through RMT benefits.
Services are delivered by qualified practitioners in accordance with their professional credentials, training, and applicable regulatory standards.
Our website and services are intended for individuals aged 18 and over, or for minors with the consent and active involvement of a parent or legal guardian.
By booking an appointment for a minor, the parent or guardian confirms they have the authority to consent on the minor's behalf and agrees to these Terms on the minor's behalf. A parent or guardian must be present during appointments involving minor patients unless otherwise agreed in writing.

Appointments may be booked online through our Jane App booking system, by phone at (416) 461-1118, or in person at our clinic. All bookings are subject to practitioner availability.
We ask that you provide at least 24 hours advance notice if you need to cancel or reschedule your appointment. This allows us to offer the appointment time to another patient.
Late cancellations made with less than 24 hours notice, or missed appointments without notice, may be subject to a cancellation fee. The applicable fee, if any, will be communicated to you at the time of booking and confirmed in your booking confirmation.
Repeated late cancellations or missed appointments without reasonable notice may affect your ability to make future bookings with us.
Please arrive on time for your appointment. Late arrivals may result in a shortened session to avoid affecting other patients. The full appointment fee may still apply.
Walk-in appointments may be available depending on practitioner availability. We recommend booking in advance to secure your preferred time.
Payment is due at the time of service unless otherwise arranged in advance.
We accept payment by the methods available at our clinic. Where you request direct billing, coverage, eligibility, and reimbursement are determined entirely by your individual insurance policy and provider. You remain responsible for any amount not covered by your insurer, including deductibles, co-payments, and amounts exceeding your annual benefit limit.
Direct billing is offered as a courtesy and is not guaranteed. We reserve the right to collect payment directly from the patient where a direct billing claim is declined or unavailable.
Prices for services are available upon request and may be updated from time to time without prior notice.
Relief Plus Wellness offers direct billing for eligible acupuncture and osteopathic manual therapy services through many major insurance providers including Sun Life, Manulife, Canada Life, Green Shield, Blue Cross, Desjardins, ClaimSecure, Equitable Life, and other participating providers.
Coverage varies by provider, policy, and individual plan. Patients are encouraged to confirm their coverage directly with their insurance provider before booking. We are not responsible for coverage decisions made by your insurer.
Wellness services are generally not covered by extended health benefit plans. Please contact your insurance provider to confirm your individual benefits.
You may be asked to complete a health history intake form before or at the time of your first appointment. The information you provide helps your practitioner deliver safe and appropriate care.
You are responsible for providing accurate and complete health information. Relief Plus Wellness is not liable for any harm arising from inaccurate or incomplete information provided by the patient or their guardian.
Any patient testimonials, reviews, or individual experiences displayed on our website or marketing materials reflect the personal experiences of individual patients and do not constitute a guarantee or prediction of results. Individual outcomes vary. Results are not typical and depend on many individual factors.

We will not be liable for any failure or delay in providing services caused by circumstances beyond our reasonable control, including but not limited to public health emergencies, pandemics, natural disasters, government-mandated closures, power outages, or other events outside our reasonable control.
In such circumstances, we will make reasonable efforts to notify affected patients and reschedule appointments at the earliest available opportunity.
In the event of a dispute arising from these Terms or your use of our services, the parties agree to first attempt to resolve the matter through good faith negotiation. If the dispute cannot be resolved through negotiation within 30 days, either party may pursue resolution through mediation or litigation in accordance with Section 15 below.
These Terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable therein. Any disputes that cannot be resolved through negotiation or mediation shall be subject to the exclusive jurisdiction of the courts of Ontario.
We may update these Terms from time to time to reflect changes in our services, legal requirements, or business practices. The effective date at the top of this page indicates when the Terms were last revised.
